Testing Accommodation Form

Attention Students:

Login to Accommodate to schedule your test or quiz with your approved academic accommodations. Select “Testing Room.” Click “New Booking Request” and fill out requested information. Check availability and select a time slot for your test or quiz. After submitting, you will receive communication from the Student Success Center (SSC) for confirmation.

Important Reminders:

• Forms should be submitted by 4:30pm at least 4 days prior to your testing day. For example, forms for Friday exams should be submitted by the preceding Monday at 4:30. If you do not submit your form at least 4 days in advance, you must contact Accessibility Services directly at accommodations@providence.edu or 401-865-2494 to discuss. You may need to make separate arrangements with your professor.

• Exams must be started and completed between the hours of 8:30am and 4:30, Monday through Friday.

• Please be aware that space in our testing room is limited. Therefore, we may not always be able to accommodate you at your preferred time. In those instances we will work with you and your professor to find a suitable alternative.

“All cell phones, apple watches, or any other electronic device will be collected before your exam and returned upon completion. If you are found to have one of these in your possession during your exam you will be asked to leave and your professor will be notified.”
